How to Become
an
Orthodontic Patient |

Thank you for considering the Case School of Dental Medicine Orthodontic Clinic as a provider for your child’s orthodontic treatment. Below is an explanation of the steps needed to become a patient in the teenage clinic. Other clinics are available for different ages and other specific orthodontic problems.
1. Call our receptionist, at 216-368-1259 to schedule a complimentary screening appointment. At this appointment, your child will be evaluated to see if he/she needs orthodontic treatment at this time and whether or not your child meets the criteria to be accepted into our program. 2. If your child qualifies for treatment, we will put his/her name on the list of patients ready to start the following fall (between July and December). You can guarantee a spot in the program by making a non-refundable $100 deposit. The $100 deposit will be credited to the total treatment fee. 3. In the spring of each year we contact the first 200 children between the ages of 11-16 who have secured the deposit and ask them to schedule a diagnostic records appointment. At this 2 hour appointment we will take an x-ray, photographs, and impressions of the teeth. These records are used to plan the course of treatment. The total fee for treatment starting in 2012 is $3100. The fee can be paid through an interest free payment plan offered by Chase Financials. 4. During the summer of each year, the residents and faculty develop a treatment plan for your child’s case. 5. Once the treatment plan is completed, you will be called to schedule an appointment for a consultation. Your resident will discuss the treatment plan and answer any questions you may have. This consultation will occur some time in the months of July through December. At this appointment you will also schedule the treatment start date with your resident doctor. Routine appointments are scheduled in the mornings on Mondays through Thursdays. We are closed on Fridays. 6. If you have dental insurance which covers orthodontic treatment, bring your insurance information on the day your child starts treatment. Again, this will be some time in the fall. We will be able to print an insurance form for you and explain the reimbursement process. Payment from the insurance company is sent to the insured. The department does not take assignment directly.
